routine home care
A patient undergoing routine home care will receive hospice care at the place she or she resides, such as a nursing home, assisted living facility, or their own home.
continuous care
Otherwise known as intensive comfort care, continuous home care consists of hospice care for a minimum of eight consecutive hours. This level of care is utilized when patients develop acute medical symptoms in their residence that require more than routine home care.
In-patient care
This level of care is utilized for the management of acute symptoms, which cannot be controlled in the home. In-patient care is to be delivered in hospitals, SNF, or sub-acute facilities.
respite care
Primary caregivers may request this level of care. This level provides brief respite--up to five consecutive days.
